Seeking individual to work directly with the Clinical Supervisor in maintaining the census at the Crossroads program. Duties include; financial assessments, work with staff from other departments within DLC to assure continuity of care for clients, court ordered registrations, group check-in and drug screens, insurance verifications, chart compliance and maintain a tracking log for state reporting. Individual must also work with family members to encourage clients treatment. This position also requires a team oriented individual to work directly with the Crossroads staff.

The individual in this position will manage all aspects of community-based case management to a mixed population. Responsibilities include treatment development and coordination of client care and services, conducting assessment evaluations, providing community linkages, assuming an advocacy role and record keeping.

Bachelors degree from a regionally accredited college or university psychology, sociology, behavioral science or related. Minimum one year experience working with both children and adults in a behavioral healthcare setting a must. Minimum intermediate-level Microsoft Office skills required as extensive electronic medical record documentation is a critical task. Valid Florida drivers license and clean driving record a MUST as extensive local travel is an essential function. Fleet vehicle or mileage reimbursement provided. Bilingual required for Immokalee location and preferred for Horseshoe location. |

A Guardian Advocate is the informed decision maker for a person who has been found by the court to be incompetent to consent to his or her own mental health treatment.
Guardian Advocates are court appointed and receive formal training. All qualified candidates that complete the training program will be included in the Center's Baker Act Volunteer program coordinated by the Baker Act Coordinator. |

A Guardian Advocate must be a competent adult who has not been: convicted of a felony; found to be incapable due to incapacity or illness, of discharging the duties; otherwise found to be unsuitable to perform the duties of a guardian; judicially determined to have committed abuse, abandonment, or neglect against a child, or found guilty or entered a plea of nolo contendere to any offense prohibited in Florida Statute 435.03; providing sustantial services to the proposed ward in a professional or business capacity, or a creditor of the proposed ward.
Individuals prohibited from serving in this capacity may not be referred to in the written Baker Act, an employee of David Lawrence Center, an employee of DCF, facility administrator, or LAC members. THIS IS AN UNPAID VOLUNTEER POSITION! |

We are seeking discreet, compassionate adults to act as healthcare proxy to individuals residing at our Crisis Stabilization Unit in the absence of a healthcare surrogate. The individual will make all healthcare decisions, including mental health, based on decisions the client would have made if competent to do so by applying substitute judgment. Some of the responsibilities would be to apply for benefits, access to the person's clinical record, authorization to release information and clinical records, and authorization to transfer to another facility. |
| Qualifications: |
Individuals who may act as proxy are guardian, spouse, adult child, parent, adult sibling, adult relative, *close friend*, or a clinical social worker.
*Close friend or Clinical Social Worker may require certain affidavits prior to performing duties of a proxy. |
